绩溪：聚力医防协同 助推医防融合创新发展

<div class="ce-paragraph cdx-block ce-paragraph--{data.alignment}">医防协同是筑牢县域公共卫生防线的核心抓手，绩溪县疾控中心锚定群众健康需求，以四大重点工作为突破，积极推动医防融合创新落地，全方位提升县域公共卫生服务质效，为健康绩溪建设注入强劲动力。</div>

<div class="ce-paragraph cdx-block ce-paragraph--{data.alignment}">创新慢病管理模式，精准筑牢健康防线。针对县域慢性病防控短板，构建“筛查-随访-干预”全链条管理体系。联合基层医疗机构开展全域健康筛查，建立高血压、糖尿病等重点慢病电子档案，实现信息动态更新。参与基本公共卫生服务、家庭医生签约等工作，实施健康积分“超市”，引导慢性病患者化“被动管理”为“主动参与”，提升慢性病管理质效。</div>

<div class="ce-paragraph cdx-block ce-paragraph--{data.alignment}">融入县域健共体改革，打通医防协同壁垒。中心主要领导兼任县紧密型县域健共体总院副院长，推进医疗机构与疾控机构深度协作，督促医疗机构落实公共卫生责任清单，推动形成“医中有防、以医带防”的健康管理新格局。牵头开展防控技能培训与应急演练，提升基层医疗机构公卫服务能力，构建上下联动的协同格局。</div>

<div class="ce-paragraph cdx-block ce-paragraph--{data.alignment}">强化健康管理师培训，夯实人才支撑根基。系统化推进健康管理师培育，采取线下集中授课和线上自学相结合的方式进行系统化培训，内容涵盖卫生健康政策、公共卫生服务、疾病防治、健康生活方式四个方面。本年度将完成70余名健康管理师培训，积极打造专业化健康管理团队，为医防融合提供人才保障。</div>

<div class="ce-paragraph cdx-block ce-paragraph--{data.alignment}">开展心脑血管监测，精准防控风险隐患。实施心脑血管疾病及其危险因素监测项目，在县域内设置监测点，覆盖不同年龄段人群。开展血压、血糖、血脂等指标检测，建立监测数据库，截至目前共完成716人次。分析监测数据精准定位高危人群，针对性开展干预服务与健康宣教，从源头降低心脑血管疾病发病风险，为防控决策提供科学依据。</div>
